Supporting Documents Required:

  • Identifications: Driver’s License, ID’s, Permits, Matricula, Passports, Foreign Driver’s License & ID’s
  • Proof Of Vehicle Ownership: Titles, Bill of Sale, Buyers Bill or Placard

Supporting Documents NOT Accepted: Car Notes, Smog Checks

 

New Policy Start Up Guidelines:

Clients must have their DL ready and Proof of vehicle ownership for Auto policies. If vehicles policy includes Comp & Collision, vehicles pictures MUST be taken.

Mendota prefers that vehicle(s) not to have any pre-existing damage, however minor damage would be acceptable with an inspection and photos of all side of the vehicle.  There would no coverage for that pre-existing damage if a claim was made after purchasing the policy.  If in doubt the agent can take photos and have them reviewed for approval if needed. The agents can attach photos in our chat feature to get a quick review.

All vehicle drivers and Co-signers must be added as drivers on the policy.

Policy purchase can be by cash, money order, or debit/credit. If policy is purchase in office, a $25 cash only in office fee will be applied. Application signatures will be sign with Xodo Sign through via E-mail or in office.

Endorsement Guidelines:

All endorsements must be established by the main insured(s) of the policy. It can be done over the phone via E-mail or in office. Signatures must be obtain under Xodo Sign. If in office, a $25 cash only in office fee will be applied when adding drivers, vehicles, or Sr22s. Supporting Documents are required.

If a Minimum Endorsement Payment Requirement is requested by Mendota, it must be paid by money order or debit/credit. Cash not accepted. 

Renewal Guidelines:

Renewals will be applied as a regular monthly payment with a debit/credit or money order. Cash not accepted. If policy is set up with EFT (Automatic Credit/Debit or, bank account), policy will renew.$20 NSF fee will be applied if the EFT payment declines. Policy will also be in risk of a possible lapse

Payment Guidelines:

Payment must be processed by a debit/credit or money order. Cash not accepted. If policy is set up with EFT (Automatic Credit/Debit or, bank account), payment will be drafted automatically.$20 NSF fee will be applied if the EFT payment declines along with a $10 late fee.

Reinstatement Guidelines:

Insured has “within” 10 days after expiration date to reinstate coverage without lapse.  Signed statement of no loss is needed.

Reinstatement Without a Lapse Guidelines:

Insured can renew up to 30 days w/out lapse; however, they may need the renewal down payment including their next installment along with a sign statement of no loss as long as no accident or violations occurred.

Backdate Policy Guidelines:

No backdates allowed.

Miscellaneous: – Unlock Quote. If you are in the final set of tabs and the application has not been signed, you can hit the
                             “previous” tab to go back and edit your quote.  If the application has already been signed you will need to
                              reach out to us so we can delete the application and unlock the quote for you.  In a
                              future enhancement (TBD) we will delete any signed applications in the overnight cycle if the policy
                              wasn’t bound.

                           – List Only Drivers List only is now an option and should be used instead of exclude in Nevada. 
                              When adding a list only household member, the List Only form (attached above) will generate and be
                              included in the E-sign process.  If the reason for list only is the person has other insurance, the agent
                              will be required to attach proof of insurance to the policy after binding.

Monthly Installment Fee: $10.00 regular payment plan, $7.00 on Credit Card Auto Pay, or $4.00 on EFT
Late Payment Fee: $10.00 only on renewals                                
Cancellation Fee: No Cancelation Fee
Reinstatement: Insured has “within” 10 days after expiration date to reinstate coverage without lapse.
                           A reinstatement fee of $40.00 and a signed No Loss document is necessary.
EFT:  Available for Bank Account withdrawal and Credit Card withdrawal ($20.00 NSF Fee)

Express Underwriting Review and Approval Procedure

 

Below is how Express Underwriting will inspect your Mendota Insurance Policy to make sure it was completed correctly and approve it for commission payment.

Mendota  Insurance

Accepts: Driver’s license, Foreign, permits, Identifications only with SR22, revoked driver’s license only with SR22 and Mendota will give our client 30 days to reinstate the Driver’s License or they will increase the premium. (But not cancel).

Application: E-sign, after it shows complete, No need to upload or email to the company, Only photos if there is comprehensive and collision, or discount proof documents, we need to download the application and have the client sign and initial the application and Express forms in Xodo Sign like we normally do. Upload Driver’s License, policy documents, and photos (if there is comprehensive and collision) to Agency Matrix, update Department of Motor Vehicle, and make notes.

Endorsement: Upload to Agency Matrix, upload documents, or photos if needed, and make notes.

Reinstate: E-sign, after is complete, download the document from the company already signed, and upload it to Agency Matrix, take payment, and make notes.

Reinstate in Renewal: If you do not get the e-sign with no loss, call the company, to avoid a lapse in coverage, in other words scenario if there is a lapse, requote the client for a backdate policy.
